Inversion is a natural form of passive gravity assisted traction (PGAT) that uses the patient's body weight in conjunction with gravity to decompress weight-bearing joints. TENS units

TENS (Transcutaneous electrical nerve stimulation) is the application of electric current produced by a device to stimulate the nerves for therapeutic purposes. In this case we are referring to portable stimulators used to help with back pain. The unit is usually connected to the skin using two or more electrodes or semi adhesive pads. Although the first units developed in the US for testing tolerance of chronic back pain sufferers to electrical stimulation in preparation for surgically invasive implants, many patients reported deriving relief from the TENS itself. A number of companies began manufacturing TENS units after the commercial success of the Meditronic device became known TENS is regarded as a non-invasive, safe nerve stimulation intended to reduce pain, both acute and chronic. Controversy does exist over its effectiveness in treating of chronic back pain. In 2007 a feature in the Pain Journal concluded that ENS was effective for chronic musculoskeletal pain whereas the journal Neurology advised against recommending TENS for chronic lower back pain,but other neurologists in the same journal suggested that in some patients TENS is useful. TENS units available from high street retailers Baxolve offers immediate relief for lower back pain through a combination of TENS therapy and lumbar support in one easy to use convenient belt.

Heat and cold back pain products

There is little empirical evidence that the application of hot and cold compresses actually effectively relieve back pain conditions, although the use of hot and cold compresses may provide relief by helping to reduce the lower back pain, spasms, and inflammation, in turn allowing for improved mobility for some back pain sufferers. Immediately after the trauma, the sufferer would apply a cold compress (generally ice packs or refrigerant gel) to the affected area of the lower back pain, maybe after a strain, for up to about 20 minutes a few time a day, for a couple of days to help reduce tissue inflammation. After a couple of days the back pain sufferer would then apply heat to the affected area, using a variety of methods such as heat pads, heat lamps etc for short durations. This has the effect of dilating the blood vessels which increase blood flow to the affected tissues, and encouraging the healing process. It also has the effect of relaxing the muscles similar to having a warm bath after exertion. As with any back pain treatment it's advisable to consult your GP before commencing a treatment.

Magnet therapy

Magnet therapy can be described as an alternative or pseudoscientific method of back pain treatment, using magnetic fields on the body to promote back pain relief and healing, with a perceived, though not understood positive effect on blood oxygen content. There is no scientific basis to conclude that small, static magnets can relieve pain It has its origins in Traditional Oriental Medicine (TOM) dating back some 2000 years, centered around 'Chi'or 'energy flows' around the body and instrumental in health and wellbeing, with magnetic wave therapy having associations with acupuncture or and acupressure. Although it is regarded with an element of scepticism due to seemingly physical and biological implausibility, in Eastern countries, therapeutic magnets are licensed as medical devices, and it does appear to be gaining some recognition within the US medical community, with an example of affirmative research of published in the MDBR Journal of Rheumatology. The downside to this is the difficulty associated with establishing true blind testing of magnets and associated 'placebo or Hawthorne' effects within test groups. By all means have a look at what's available but remain sceptical.

Ultrasound

Ultrasound is often used as a method of pain relief by physios and therapists in combination with other pain treatments, such as manipulation, massage, ice treatment and interferential (electrotherapy). The ultrasound device causes millions of sound waves to penetrate deep into tissue in order to promote healing of inflamed or damaged muscles, tendons and ligaments. It's claimed that ultrasound can reduce tissue inflammation by stimulating blood flow, promote healing by stimulating soft tissue, and assist in the absorption uf analgesics, and reduce pain. Again there is conflict over the effectiveness for lower back pain treatment with both positive user testimonials offset by inconclusive meta analysis. Ultralieve Pro is a mains operated or portable hand held medical grade device designed for home use to help with pain and promote healing by conducting ultrasound waves into the damaged tissue. Although the device is easy its perhaps best if the user has received ultrasound treatment previously from a physio or healthcare professional, and again it's advisable to check with your GP or physio before using one to derive pain relief It should not be used as a replacement for conventional help with back pain but in conjunction with it in between sessions. Spinal Stenosis Understanding


Spinal stenosis can be a pain in the neck, literally! This condition is the result of a gradual narrowing of the spinal canal, which may place pressure on the spinal cord or nerves. This most commonly affects the lumbar (low back) and cervical (neck) areas of the spine. The most common cause of spinal stenosis is aging. The rate of progression of the disease varies as people age and the changes are often quite gradual.

Causes of Spinal Stenosis
Spinal stenosis is usually a natural progression of aging but it can sometimes be caused by the herniation of a spinal disc. It may also be due to arthritis developing in the vertebral joints which decreases the space for the nerve roots which branch off the spinal cord. Paget's Disease, characterized by enlarged and deformed bone growth often affects the spine, causing stenosis.

Lumbar Spinal Stenosis Symptoms
The degree of symptoms vary from mild low back discomfort to a complete inability to walk.


  • If the lower portion of the spinal cord is compressed, there is often weakness and pain in the buttock or leg. The pain may be due to compression of the nerve itself or because of compression of blood vessels carrying nutrients and oxygen to the nerve. Each of these processes interferes with the natural function of the nerve roots and decreases the endurance and effectiveness of the spinal nerves. Both cause pain.

  • Mild lower back pain may progress to more severe pain in buttocks, thigh or leg that develops with walking and standing. The pain decreases with rest.

  • Numbness and weakness in the lower leg may lead to a total lack of sensation.

  • Some patients will have leg pain and numbness without any back pain.

  • Positioning relieves some symptoms as they increase the amount of space for the spinal nerves. Flexing the lumbar spine and bending forward often helps.

  • A patient with spinal stenosis can ride a bike, walk up a hill or a flight of stairs without having any pain. They can often walk for extended distances if they have something to lean on, such as a shopping cart.

  • Walking down a hill, an incline, or a flight of stairs or when the patient must walk without the cart, causes the symptoms to reappear.

  • Nerves to the bladder and bowel may become compressed and can lead to partial or complete incontinence.


Cervical Spinal Stenosis


  • Pain in neck can be mild, at times, becoming more severe

  • Headaches

  • Pain in the neck and shoulders

  • Numbness or weakness in the arm or hand

  • Balance problems and clumsiness


The Severity of the Symptoms depends upon several factors:


  • The original width of the spinal canal

  • The sensitivity of the involved nerves

  • The pain tolerance of the patient

  • The physical demands of the patient's lifestyle


Diagnosing Spinal Stenosis


  • Complete history to determine how long symptoms have been present and if condition is worsening

  • Physical Exam to evaluate pain and weakness of extremities

  • Neurological exam will identify chronic nerve root depression

  • Laboratory tests to rule out unusual causes of nerve root and spinal cord dysfunction

  • X-rays of lumbar and cervical spine can identify the amount of spinal degeneration or instability.

  • A CAT scan (Computerized axial tomography) helps visualize the spinal column and identifies areas of stenosis.

  • A myelogram, in which a dye is injected into the space occupied by the spinal cord and nerve roots, can identify problem areas.

  • An EMG (electromyographic) test helps identify compressed nerve roots.

Most patients respond well to non-surgical treatment, medication and a physical therapy regimen.

How Much Does Back Pain Cost Us?


Acute or short-term lower pains in the lower back generally lasts from a few days to a few weeks. Most acute pain is mechanical in nature - the result of trauma to the lower back or a disorder such as arthritis. Pain in the lower back from trauma may be caused by a sports injury, work around the house or in the garden, or a sudden jolt such as a car accident or other stress on spinal bones and tissues.

Symptoms may range from muscle ache to shooting or stabbing pain, limited flexibility and/or range of motion, or an inability to stand straight. Occasionally, pain felt in one part of the body may "radiate" from a disorder or injury elsewhere in the body. Some acute pain syndromes can become more serious if left untreated.

Chronic back pain is measured by duration - that which persists for more than 3 months is considered chronic. It is often progressive and the cause can be difficult to determine.

Incidence of Back Pain

According to the most recent back pain data published on the American Academy of Orthopaedic Surgeons website, there were some 13 million reported physician visits resulting from pains in the back. Women affected by backache made up 7.1 million, whereas male backache sufferers numbered 6.2 million.

There were an additional 8.8 million people who specified pain in the lower back at physician visits. Patients ages 43-61 (41%), were most likely to report backache at visits, representing 5.4 million patients. Backache visits among 19-42-year-olds represented 3.6 million visits (27%) and patients older than 61 reporting back pain represented 3.7 (28%) million visits.

Work related injuries represented 251,000 back pain patients and 123,000 lower back patients.

Lost Productive Time Due to Common Pain Conditions in the US Workforce

It was estimated that nearly 13% of the US workforce experienced a loss in productive time during a two-week period due to a common pain condition:

5.4% for headache

3.2% for backache

2.0% for arthritis pain

2.0% for other musculoskeletal pain

Cost of Lumbar Therapy

In 2005 Americans spent $85.9 billion looking for relief from back and neck pain through surgery, doctor's visits, X-rays, MRI scans and medications, up from $52.1 billion in 1997, according to a study in the Feb. 13 issue of the Journal of the American Medical Association (JAMA). That money hasn't helped reduce the number of sufferers; in 2005, 15 percent of U.S. adults reported back problems-up from 12 percent in 1997.

Not only are more people seeking treatment for back pain, but the price of treatment per person is also up. In the JAMA study, researchers at the University of Washington and Oregon Health & Science University compared national data from 3,179 adult patients who reported spine problems in 1997 to 3,187 who reported them in 2005-and found that inflation-adjusted annual medical costs increased from $4,695 per person to $6,096.

Spinal patient costs were also significantly higher than for non spinal patients. It was reported that people with back pain problems cost 76 percent more on average than people without back problems each year.

Glucosamine for Joint Health and Repair


Glucosamine is an amino acid and sugar (glucose) combination. It is essential for the maintenance and repair of joints and is naturally produced in the body. As we age the body's ability to produce glucosamine from glucose decreases because the quantity of the enzyme glucosamine synthetase, responsible for the process declines. After an injury to joints, cartilage or spinal discs the body is often unable to make sufficient glucosamine for healing to occur.

Glucosamine acts as a supportive building material and stimulates the production of other ingredients essential for cartilage and joints. Research has shown conclusively that glucosamine considerably alleviates painful, tender and swollen joints. There is also a marked improvement in joint mobility in arthritis sufferers. In some studies there was also a reversal of cartilage degeneration. Taking glucosamine as a supplement can effectively inhibit cartilage deterioration. Clinical studies have shown that glucosamine sulphate relieves the pain and inflammation of osteoarthritis It is a safe alternative to non-steroidal anti-inflammatory drugs (NSAIDs eg ibuprofen). Although NAISDs are able to provide quick relief they have been shown to inhibit repair and accelerate the degeneration of the cartilage. Glucosamine achieves very positive results but it takes several weeks- so you need to be patient!

Glucosamine sulphate is the first line of treatment for arthritis - taking precedence over conventional medication in several European countries. There are no known side effects and no contraindications for its use - diabetics need to be careful as glucosamine might have an effect on insulin resistance.

Although the exact mechanisms for alleviating osteoarthritis symptoms is not known research suggests that glucosamine encourages cartilage matrix formation, reduces enzymatic break-down of cartilage components and had an anti-inflammatory effect. It also stimulates the production of hyaluronic acid in joint fluid. Hyaluronic acid relieves pain and improves mobility by repairing damaged cartilage.

There are many research studies into the benefits of glucosamine in arthritis. In a double-blind study 178 osteoarthritis patiens reported the effectiveness of glucosamine sulphate in reducing pain. In another study 252 patients with osteoarthritis of the knee received 1,500 mg per day of glucosamine sulphate in three doses. After 4 weeks glucosamine sulphate was more effective than the placebo in reducing pain and increasing mobility.

As well as having a role in treating arthritis glucosamine can be used following sporting injuries. After trauma or tearing during strenuous activity, the body often cannot make enough glucosamine for optimal healing. In one study 68 athletes with knee cartilage damage were given 1,500mg glucosamine sulphate daily for 40 days, then 750mg for 90 - 100 days. 52 of the athletes had complete resolution of the symptoms and returned to full training. Follow up after 12 months the athletes showed no signs of cartilage damage.

As well as glucosamine there are other substances that can play a role in joint health. These include:

· MSM (methyl sulfonyl methane) - a natural, safe and physiologically active sulphur compound. It has ano-inflammatory and antioxidant action. It is found in our bodies and in some foods such as raw seafood and fruit and vegetables. It plays a role in creating new cells and stimulating healing. It is used in the treatment of all forms of arthritis and joint infections.

· Chondroitin - is another important component of cartilage. It acts as a shock absorber by providing structure, holding water and nutrients and allowing other molecules to move through the cartilage. Glucosamine and chondroitin work together to protect the cartilage against degeneration.

· Vitamin C and manganese also work to strengthen the body's natural healing mechanisms.

· Multivitamin and mineral supplements and essential fatty acids also have beneficial effects on joints and musculoskeletal problems.

I'll admit, I was skeptical at first. Back Suffering - What Is Osteophyte?


Osteophytes are little round swelling of more bone that grows around the joints. They are sometimes called bone spurs. The first chakra is the root chakra or the Kundalini chakra. It is located at the base of the spine. The root chakra governs our body's survival mechanism, such as food, shelter, and security. Our relationships in early childhood, especially those which are associated with emotional and/or physical well-being are influenced by this chakra. It stimulates activity, exercise, and vitality, as well as our general health throughout life. The root chakra also relates to our material and financial achievements, success in business, and our status in society. Spinal Decompression For Herniated Discs


Spinal decompression for herniated discs is a modern approach to an ancient back pain treatment modality. Decompression is a non-surgical therapy which demonstrates very good curative results for herniated and degenerated intervertebral discs.

Decompression is a growth sector within the established back pain treatment industry. Many orthopedists, chiropractors and other types of care providers are investing themselves in the latest technology. Decompression generally takes about 1 month to complete. The patient will often have to go to the care facility for treatment almost every day for the duration of the therapy. Most treatment sessions take 30 to 45 minutes and may be supplemented by additional practices, such as TENS, hydrotherapy, ultrasonic heat or massage. Once the treatment is complete, the patient should not have to return for treatment in the future, but some patients may find an occasional touch up may reinforce the permanency of the results.

Chiropractic Care as an Alternative for Arthritis Since I'm a chiropractor, you know I have to start here!

Back and Joint Pain-New Relief Through Lipoxygenase Inhibition With Natural Supplements


Lipoxygenase Inhibition: A Missed opportunity for controlling pain and inflammation

Are we only "half treating" our back and joint pains?

Back and joint pains are among the top reasons for visits to the doctor. Yet complete resolution of the complaint is often slow in coming or never completely resolved. Why?

There are 2 major physiologic pathways leading to inflammatory and pain responses: the cyclooxygenase (COX) mediated and the lipoxygenase (5-LO) dependent one. While the former is widely known, and inhibited by the well popularized NSAID's (like Celebrex, Vioxx(!)), etc, the second one is virtually ignored in the current management of pain syndromes. Principal cause for this is the failure of major drug companies to develop synthetic drugs that can inhibit the 5-LO and its downstream metabolites, the all important leukotrienes. To make up for the failure to deal with the 5-LO, pharmaceutical solutions like corticosteroids are usually put in place - with their well known side effects.

The bottom line is that when only one of two pathways to pain is addressed the patient is only "half treated" and half satisfied.

Lipoxygenase and pain

The 5-LO enzyme works to produce the "misery" of the leukotrienes.1 They are abundantly involved in over 35 chronic conditions including: asthma, allergies, colitis, arthritis, gastric disorders (promote ulcer formation, stimulate acid secretion, etc), scleroderma, neurological diseases, and so on. 2

More recently the involvement of the leukotrienes in pain syndromes has become clear from a multitude of studies. 1

5-LO and leukotriene B4 are involved in orofacial pain perception and mechanical and thermal sensitivity.3-7

Postoperative incision pain in animal models could be considerably reduced using experimental 5-LO inhibitors.8

The beneficial effects of 5-LO inhibition were demonstrated in the reduction of inflammatory events accompanying experimental spinal cord injury. 9

Several studies have identified inflammatory mediators in disk herniation, such as leukotrienes. Cytokines occurring in degenerated facets have been shown to contribute to the pain of degenerative lumbar disorders. 10, 11

5-LO has been shown to be involved in both pain modulation and induction of opioid tolerance at the spinal level. 12 5-LO metabolites are found in clinical cases of herniated nucleus pulposus and experimental data gathered in the study of associated radicular pain in animals demonstrated that 5-LO inhibition may prove to be beneficial in such conditions.13

Pharmacological inhibition of the 5-LO

While the market availability of COX inhibitors is widespread the opposite seems to be the case with pharmaceuticals in the lipoxygenase class direction. Partially this is not due to lack of trying. Promising experimental drugs had to be abandoned due to unacceptable side effects - death of animal subjects! Even those that made it to market carry warnings of hepatotoxicity (Zileuton) or have been associated with an increase in abnormal mental behavior (Singulair). On the other hand there is a persistent lack of research on the part of the pharmaceutical industry secondary to a tragic underestimation of the potential market size.

Financial disincentives explain the lack of studies of extracts of "natural" substances which are not easily patentable.

Advances in nutritional therapy with high concentration boswellia (frankincense) extracts

The premier 5-LO inhibitor is the natural, herbal ingredient AKBA, acetyl-11-keto-beta-boswellia acid, the most active component of the frankincense, Boswellia serrata. Boswellia as such has been known for centuries to be a potent anti-inflammatory agent. Studies have proven its efficacy in arthritis, colitis, allergies and environmental sensitivities. 14-16

More recent studies have confirmed the analgesic properties of boswellia extracts, both as stand alone solutions as well as synergistic enhancers of pain relief when given in conjunction with COX inhibitors, opioids and other NSAID's. 17, 18

The success of boswellia extracts is all the more surprising since only poorly standardized products have been available on the general market. The component AKBA is recognized as the active anti- inflammatory principle in the boswellia and yet by far most of the formulas have only 1-3% AKBA concentration.

Fortunately high concentration boswellia extracts are now available with a concentration of over 90% AKBA! This leads to enhanced efficacy. There are a number of high quality boswellia products on the market. To get the best a careful reading of the supplement facts on the label is necessary.

If the label does not specifically state that the AKBA content is minimum 90% you are not getting the best quality possible.

With the right quality and dosage, however, either taken alone or together with other therapies symptom relief could be seen from this nutritional modality. Improvement in previously therapy resistant back or joint pain, prolonged "holding" of chiropractic adjustments and faster pain relief after injury has been routinely noted.

Safety and toxicology;

High concentration boswellia extracts are considered GRAS - generally regarded as safe. There are practically no side effects except for an occasional report of headache. There have not been any reports of the intestinal distress seen with other boswellia preparations.

Conclusion

High concentration boswellia extracts with 90% or more AKBA is highly beneficial for the treatment of pain syndromes ranging from back to joints and other damaged soft tissues. They can help in other organ and neurological pain conditions due to their anti- inflammatory properties. It can be given as a stand alone solution or in conjunction with other COX inhibitors. They are considered nutritional supplements and should be part of a general health maintenance.

How to Establish Good Posture for Life


Good posture isn't about good manners anymore; it's about establishing a healthy lifestyle. Poor posture can become permanent. Indeed, the unhealthy habits of youth can inform the body's growth for years to come. Good posture is not necessary given the respect it deserves in today's popular culture.

Let's break "posture" down
The form our bodies take while standing, lying down, and sitting is what we call posture. Muscle tension versus gravity - when the body can properly support itself, in alignment, good posture is possible. We control our bodies through posture, which is essentially the employment of muscles that helps us to be upright.

We are not always aware of our posture; our bodies can rely upon muscle memory to propel ourselves. The two muscle groups most responsible for healthy posture are the lower back and the hamstrings. The skeleton, which is held together by ligaments, still needs the muscles to resist gravity. Ligaments are, themselves, too weak to carry out movement alone. Good posture requires suitable muscles and joints: you must be strong enough throughout your body, but particularly in the muscles surrounding your vertebral column. In addition, all of your joints should be able to operate through a full range of motion. Mindfulness is also key, as you must maintain awareness as your body slips in and out of viable, stable postures.

The drawbacks of improper posture:
You can unnecessarily strain your muscles with poor posture; conversely, a particular stance, when held for a long time, may relax them. Stiff muscles, wearing high-heeled shoes, stress, pregnancy, obesity and weak postural muscles are common causes of poor posture. You can also get bad posture from habitual poor posture situations at work or sitting and standing, or simply low flexibility.

Ideal sitting practice:

  • Rest your feet either on the floor or on an object like a foot rest at an appropriate height.

  • Don't let yourself cross your legs, and make sure you knees are behind your ankles.

  • Reserve a little space for your knees - don't sit so far back that they abut the seat.

  • Your knees should stay level with or under the height of your hips.

  • If you don't receive adequate mid and low back support from your chair, try slipping a pad or pillow behind you.

  • Forearms parallel to the floor, permit your shoulders to relax.

  • Intermittently, make a change in how you're sitting.

Ideal standing posture:

  • Your weight should rest firmly on the balls of your feet, for the most part.

  • Your knees should keep a gentle bend.

  • At shoulder width apart, your feet should be planted firmly.

  • Your arms should be at your sides, hanging effortlessly.

  • You should stand straight and tall, your shoulders held back.

  • Your stomach should be tucked in.

  • Your head should keep a position of delicate balance, centered, with the earlobes in line with the shoulders. This keeps your muscles from unnecessarily supporting the weight of your head.

  • If you find yourself standing for a great deal of time, be aware that you should shift your weight back and forth between heel and toe, as well as between your left and right feet.

Can I still correct posture problems at my age?
You can always fix poor posture, though for better or for worse, your joints determine their shape and function based on their surroundings. This means that it can take as long or longer to fix bad posture as it did to acquire it.
